I, the undersigned parent/guardian of the above child, acknowledge and agree to the following rules and regulations:
- Fees are payable on time and according to the school's arrangement.
- I grant permission for my child to be transported by school staff where appropriate.
- I authorise the principal/centre manager to act on my behalf in emergencies, including administering medicine or procuring medical care where reasonably necessary.
- I will notify the centre of any change of address or other important information without delay.
- Where I provide details of other people (a partner/spouse or authorised pickup persons), I confirm I have informed them that their information will be shared with the school for enrolment and child-safety purposes.
